Overview

Total Health Connections represented a significant evolution of Elevance Health's integrated clinical offerings, building upon the foundation established by Total Health Total You. THC brought care management, behavioral health, disease management, medical benefits management, maternity and family health, and other established clinical services into a more coordinated offering supported by dedicated Family Advocates who helped members navigate the resources available to them.

Successfully launching the offering required far more than delivering technology. It required aligning clinical operations, technology platforms, reporting, member engagement, and implementation processes to deliver a seamless integrated experience for members, clients, and care teams.

The transformation built upon an integrated model already serving more than 4 million members across 100+ clients and was supported by a broader enterprise advocacy investment exceeding $13 million.

My Role

A critical part of the launch was ensuring that significant changes behind the scenes remained seamless to clients while new elements of the Total Health Connections experience became visible from day one. As existing clinical programs transitioned from legacy systems to a new enterprise medical management platform, I worked across clinical operations, engineering, data integration, and enterprise reporting to preserve continuity in how engagement and program performance were measured. Clients already knew these programs and relied on established reporting, so an internal technology migration could not disrupt the clinical value story they had been receiving.

At the same time, the new Family Advocate model introduced interactions that had never existed in the reporting ecosystem. I worked closely with operations and engineering to understand the proposed workflows, model the expected data using diagrams and simulated interactions, and define how Advocate activity would move through enterprise data integrations and ultimately into Client Information Insights. The goal was not simply to move data successfully, but to connect it correctly to the broader clinical experience so Advocate engagement could be accurately represented from the first day of launch.

The Opportunity

Many of the first Total Health Connections clients were not new to Elevance Health's clinical programs. They were existing customers upgrading from earlier integrated offerings to a more comprehensive—and higher-cost—model. Reporting therefore had to accomplish two things at once: preserve continuity in how clients understood the value of their existing clinical programs while also demonstrating what they were gaining through Total Health Connections.

That created a particular challenge immediately after launch. Meaningful clinical outcomes take time to emerge, but clients needed early evidence that the enhanced model was delivering additional value. Initial reporting therefore had to make the new connection points visible—Family Advocate engagement, referrals into clinical programs, navigation activity, and other indicators showing how members were being connected to available care and resources. Because the dedicated Advocate relationship was central to the Total Health Connections value proposition, accurately capturing and reporting those interactions from the first quarter was critical to demonstrating not simply continued value, but the value of the upgrade itself.

Connecting Strategy to Execution

Much of the reporting design had to be completed before real production data existed. Clinical workflows were still being finalized, so reporting logic and data integrations were built using workflow diagrams, expected system behavior, and simulated test data. That required designing flexible integrations that could accommodate differences between expected and actual production behavior without disrupting enterprise reporting.

Deep familiarity with the underlying clinical programs became a critical advantage throughout the initiative. Because I understood how care management, behavioral health, maternity, medical benefits management, and Family Advocate workflows were intended to operate, I could validate whether the data accurately reflected real clinical engagement rather than simply confirming that system integrations were technically successful.

When production data became available following launch, the initial reporting assumptions could be compared with actual operational behavior. One Family Advocate interaction pattern differed from the simulated workflow, requiring rapid adjustments to preserve accurate engagement reporting while maintaining continuity for clients transitioning from Total Health Total You.

Results

The launch established the reporting and data foundation needed to preserve continuity across existing clinical programs while making the new Total Health Connections engagement model visible to clients. Early reporting could demonstrate Family Advocate engagement, navigation, and connections to clinical resources while longer-term clinical outcomes developed over time.

The initiative reinforced that successful digital transformation extends well beyond implementing new technology. It also requires preserving the integrity of the data organizations rely on to measure performance, demonstrate value, and maintain trust throughout periods of significant operational change.

Successful digital transformation depends as much on preserving trust in the data as it does on delivering the technology.
